GoldCap Liability Waiver
By registering my minor for programming with GoldCap Education LLC, I acknowledge that myself and child are voluntarily engaging and participating in GoldCap Education Programming, I am agreeing to the agreement below and to follow guidelines, rules, behavior expectations and directives provided by GoldCap Education staff.
By registering for GoldCap programming, I hereby agree to release, indemnify, and hold harmless GoldCap Education LLC and its staff from any and all claims, demands, actions, or causes of action arising out of or relating to any loss, damage, or injury, including but not limited to bodily injury, property damage, or loss, which may be sustained by the participant during or as a result of participation in GoldCap programming.
GoldCap is committed to providing a safe and inclusive environment for all that participate. In order to meet these goals, GoldCap expects all participants to treat each other, staff members, and property with respect and kindness. This includes using appropriate language, managing physical boundaries, and listening to instructions from staff members.This includes staying within designated areas, using technology or equipment properly, and reporting any unsafe situations to a staff member immediately.
Discrimination, bullying, or exclusion of any kind will not be tolerated. Participants are responsible for their own actions and behavior. This includes taking care of personal belongings, responsibility for GoldCap property or property owned by space used by GoldCap, and the property of other participants.
By registering for GoldCap programming, I am providing consent for my childs image to be used for GoldCap advertising and social media purposes. All images will be used in good faith, and feature mostly group settings. Parents have the right to notify GoldCap administration at in**@go********.com to ask their child not be featured in any media used publicly.
I acknowledge and understand that while GoldCap will make every effort to mitigate risks associated with dietary allergens for my child, I am responsible for educating my child about avoiding risk related to allergens and GoldCap will not be held responsible for any exposure or reaction to food or other allergens during participation. I understand that GoldCap makes every effort to provide a safe environment for all students, including those with dietary restrictions or allergies. However, GoldCap does not have standing restrictions on foods or allergens. Participants participate at their own risk and assume liability for any food or allergic reactions.
I acknowledge that participation may involve certain risks, including but not limited to:
1. Physical activities such as walking, physical group activities, or other activities that involve movement and physical participation.
2. Educational activities involving technology, tools, equipment, or materials.
3. Interaction with other participants, curriculum, and GoldCap staff.
4. Exposure to allergens, food or otherwise.
I understand and accept these risks, and agree to assume full responsibility for myself and my student for any injuries, damages, or losses that may occur as a result of participation in GoldCap Programming. I understand that this waiver and release of liability is binding upon me, my child or student for whom guardianship is held, and that it shall be governed by the laws of Iowa City, Johnson County and the state of Iowa.
I have read this liability waiver and fully understand its terms. I voluntarily agree to its contents and acknowledge that I am waiving certain legal rights by registering. Registration in GoldCap program confirms agreement to the terms and conditions listed above.
